A.  Active Care Only Program

S4Success™ team provides the same support services and software as Full Service Program for the period spanning from clients’ intake to discharge.

B.  After Care Only Program

S4Success™ team provides the same support services and software as Full Service Program for the period spanning from discharge up to one year thereafter. This program excludes the linked tablets.

Providers Role Consists in:

  • Providing access to S4Success™ EMR System to set up clients’ profiles.
  • Pulling S4Success™ Software Generated Reports as needed for clinical outcomes, utilization reviews, and management tuning.
  • Scheduling clients with S4Success team for test administration.
